Obesity has emerged as a major global healthcare problem with cardiovascular disease representing the leading cause of mortality in this population. Obesity is causally linked to the development of insulin resistance, diabetes mellitus, micro- and macro-vascular disease, and atherosclerosis. Adipose tissue dysfunction with overproduction of proatherogenic cytokines and adipokine dysregulation contribute to establish a low-grade systemic inflammatory state that is strongly implicated in these disease processes. This chapter will review how the adipose tissue microenvironment is associated with microvascular dysfunction characterized by abnormalities in vasodilator and angiogenic functions, which may contribute to mechanisms of adipose tissue dysfunction and consequently whole-body cardiometabolic disease.
